Montachusett Home Care Corporation, a progressive elder service agency seeks candidate to serve as Functional Skills Trainer (FST) in the Personal Care Attendant (PCA) Program. This new opening is a part time opportunity (17 hour work week).

Qualifications: Experience working with elders and the disabled population desirable Candidates with bilingual skills are preferred. Computer experience: typing and data entry skills are necessary. High School diploma is mandatory. An Associates’ Degree and/or license in social work is/are preferred. A valid driver’s license along with a fully registered and insured automobile is required.

Responsibilities : Interacting with potential PCA consumers via the telephone followed by home visits to introduce the program.
Coordination of needed medical paperwork and training with consumers to self direct their services are key components to the PCA program. Concise documentation and detailed instruction for consumer and surrogate (if consumer cannot successfully self direct) are important also. Updating the nurse on the status of pending PCA consumers and trying to obtain timely program approval are key goals. For active PCA consumers, FST to plan and complete follow up home visits and document efforts in consumer’s records.
