LEOMINSTER_Susan Hubert of Gardner was nominated for the Cathe Madden Aging Services Access Point (ASAP) Case Manager Award for 2017.  This prestigious award is presented by the Home Care Aide Council at its annual celebration each year in Randolph, MA.  The Home Care Aide Council is the professional association that represents the vendor agencies that employ home care aides across the state.   Nominations for the Cathe Madden Award are made by staff of these agencies that have direct contact and working relationships with case managers employed by the 25 ASAPs in the Commonwealth.             Susan has been employed by Montachusett Home Care Corporation (MHCC) of Leominster for 16 years.  ASAP case mangers and the home care aides work closely together to ensure services provided to consumers are meeting the needs of the elder and that care plans and the scheduling of services are adjusted when necessary to respond to an elder’s changing needs.  Montachusett Home Care Corporation is a not for profit agency located in Leominster whose mission is to enable elders and disabled individuals to remain safely in their community through the provision of appropriate services and to prevent unnecessary nursing home placements.  Our agency serves 21 communities in north central Massachusetts.  We can be reached at 978-537-7411.
