The Executive Office of Elder Affairs (EOEA) contracts with Aging Services Access Points (ASAPS) throughout the Commonwealth of Massachusetts, to administer homecare programs for persons sixty years of age and over. Montachusett Home Care (MHCC) is a private, non-profit agency, serving as the ASAP in 21 cities and towns in North Central Massachusetts. MHCC is seeking services for the period of October 1, 2021 – September 30, 2024.
Please be aware that services with titles starting with “COVID” are temporarily being offered due to the COVID-19 pandemic and will end in accordance to EOEA guidance.
MHCC will determine consumers’ eligibility for services and service schedule. MHCC enters into agreements directly with provider agencies, who provide the direct services to consumers. MHCC does not commit to award a provider agreement or to pay for company costs incurred in this proposal. MHCC reserves the right to request more information or call for new proposals at any time during the process. MHCC negotiates rates within the Executive Office of Health and Human Services (EOHHS) and EOEA guidelines, and reserves the right to accept or reject any proposal. Before issuing a provider agreement, MHCC must receive the following documents: a copy of your certificate of insurance, Form W-9, a completed Administrative Overview, completed Service Specific Attachment(s) for all services you wish to provide, and all other associated documentation requested in this application.
Bids from Minority Business Enterprises (MBEs), Women Business Enterprises (WBEs), Minority and Women Nonprofit Organizations (M/WNPOs), Veteran Business Enterprises (VBEs), Service-Disabled Veteran-Owned Business Enterprises (SDVOBEs), Disability-Owned Business Enterprises (DOBEs), and Lesbian, Gay, Bisexual and Transgender Business Enterprises (LGBTBEs) are highly encouraged. Homemaker/Personal Care providers must complete/update the Notice of Intent (NOI) process and received approval through EOEA’s website: NOI 800 Age Info, prior to completing this MHCC provider application.
State Home Care providers are now required to enroll in the MassHealth Frail Elder Waiver Program and receive a provider number, as vendors may provide services to consumers enrolled under Frail Elder Waiver Programs.
