We offer a variety of caregiver and support services so that older people, individuals with disabilities and their families are empowered to make personalized choices to ensure a life of dignity, safety and respect.

Home Care Services
Through our array of home care programs, we provide a variety of services to eligible elders including, but not limited to homemaking, personal care, home-delivered meals and health-related services to enable them to remain safely in their own home. We also offer consumer directed care options for elders and disabled persons in which the consumer is trained to hire and manage their own care providers. In addition, we provide a range of home and community-based services to disabled adults enrolled in both the Medicare and Medicaid programs.

Caregiver Support
Our Caregiver Coordinator is available to help caregivers of frail elders with counseling, information on available services, referrals to those services, a support group, and scholarships that provide needed support and relief to caregivers.

Adult Family Care
Matches elders and disabled adults 16 years of age and older with host families who provide a family-like environment, daily meals, and assistance with or supervision of daily personal care. Family members (except spouses) can be paid caregivers.

Supportive Housing
Supportive housing is public housing for elders and disabled individuals with on-site services, including a daily meal and 24 hour emergency care at an affordable cost. We currently offer this program at three sites: Gardner, Fitchburg and Leominster.

Protective Services
For anyone 60 years of age or older who is physically, emotionally and/or sexually abused, financially exploited, neglected by a caregiver, or who is neglecting their own needs, MHCC will intervene to determine the extent of the abuse or neglect and provide services to eliminate or alleviate the problem. We also offer assistance to elders in other crisis situations

Congregate Housing
A shared living, but home-like environment, that offers privacy yet promotes opportunities for socialization. Residents are encouraged to live as independently as possible and yet, secure in knowing that home care services are available should they need assistance from others.

Money Management Program
The Money Management Program is a bill paying and budgeting service that ensures regular household bills are paid in a timely manner. This service is a critical support to those who struggle with physical disabilities, confusion or mental health issues that make bill paying difficult. Using a combination of paid and volunteer staff, we ensure essential household bills are paid, allowing consumers to achieve economic security, maintain independence and peace of mind.
SHINE (Serving the Health Insurance Needs of Everyone)
The SHINE Program provides free health insurance information, counseling and assistance to elders and disabled persons receiving Medicare. Our trained counselors help Medicare recipients better understand their benefits under this program, as well as their other health insurance options.
